What is Making Care Primary (MCP)?

Making Care Primary (MCP) is a revolutionary initiative to deliver personalized, value based patient care. The Centers for Medicare & Medicaid Services (CMS) launched MCP to create new healthcare service delivery methods. Starting in June 2024, the 10.5 year MCP program gives primary care providers everything they need to care for their patients.

Through our partnership with Medicare, we are thrilled to offer MCP to our patients at AZZ Medical Associates. A Progressive Model with Three Tracks

The Making Care Primary Model includes three progressive tracks to match primary care providers based on their value-based care expertise. The value-based program consists of three tracks that help providers grow their skills and improve whole person care for patients as they move from basic payment methods to advanced payment systems.

Track 1: Building the Foundation

The entry level track helps primary care providers develop a value-based care foundation through 2.5 years of program work.

Focus: Healthcare teams identify high-risk patients, work with better systems, and create a framework to control long-term diseases.
Payment: Participants receive Fee-for-Service (FFS) payments as they develop their care structures with financial assistance provided by Medicare.

Track 2: Advanced Primary Care Implementation

Providers gradually develop their services over the next 2.5 years by introducing full scale chronic disease and integrated care systems.

Focus: Providers focus on behavioral health testing by partnering with government support groups to improve patient care.
Payment: Population-based payments alongside FFS help providers get better results while advancing healthcare delivery methods.

Track 3: Optimizing and Sustaining Excellence

The final track accumulates to 5.5 years, helping providers develop high quality integrated care delivery capabilities.

Focus: Healthcare teams focus on workflow improvements, combining better teamwork with medical specialists and community partners to provide complete health treatment to patients.
Payment: Payment systems based on patient outcomes give quality rewards and help healthcare stay stable.
