La-Toya Newsome, MSN, RN, ANP-BC, is an experienced Nurse Practitioner with a strong background in Primary Health Care, Emergency medicine, and therapeutic interventions. She has recently joined AZZ Medical Associates as a dedicated provider, bringing her expertise and commitment to delivering exceptional patient care. La-Toya’s professional experience includes working at reputable healthcare organizations, such as Davita and UnitedHealthCare (OPTUM) Long Term Care Program. At Davita, she has been providing comprehensive assessments and ensuring compliance with Medicare and Medicaid guidelines. She functions autonomously and collaborates effectively with the healthcare team to coordinate and facilitate high-quality care for patients with end-stage renal disease (ESRD) and chronic kidney disease (CKD). La-Toya also conducts telehealth and in-person visits to ensure the continuity of care. During her tenure at UnitedHealthCare (OPTUM) Long Term Care Program, La-Toya J Newsome effectively managed both medical and behavioral chronic and acute conditions. She delivered cost-effective, quality care to assigned members, performed comprehensive assessments in compliance with CMS guidelines, and accurately diagnosed and supported visits with ICD 9/ICD10 codes. She also played a crucial role in medication reconciliation, utilizing practice guidelines and reviewing orders and interventions for effectiveness. Prior to that, La-Toya served at Henry J. Austin Health Center, where she managed a panel of patients and provided primary, secondary, and tertiary care. She conducted comprehensive and episodic medical assessments, advised patients on lifestyle changes and medication adherence, and determined the need for consultation or emergency department evaluation when necessary. La-Toya also provided urgent care for various conditions and comprehensive care to patients with chronic or disabling medical conditions. In addition to her clinical experience, La-Toya has held supervisory and leadership roles, such as Shift Coordinator/Nurse Manager at Palisades Medical Center and Nursing Home Supervisor at Fountains of the Manor. She obtained her Bachelor of Science in Nursing (BSN) degree from Bloomfield College. La-Toya is a licensed and board-certified Advanced Practice Nurse in both New Jersey and New York. She maintains active registration as a Registered Nurse in both states. Additionally, she holds various certifications, including Advanced Cardiovascular Life Support (ACLS), Pediatrics Advanced Life Support (PALS), and Basic Life Support (BLS). La-Toya is also trained in Intravenous therapy and is authorized to prescribe controlled substances (CDS).
